A qualifying life event is a change in your circumstances, such as marriage, divorce, and disability, which may impact your health benefits. Notify the Plan immediately when you experience a life event.

The following are examples of common life events and other changes that may affect your health coverage:

• Marriage
• Divorce
• Birth of a new child
• Adoption or placement for adoption
• Legal guardianship
• Physically and/or mentally disabled dependents age 26 or older
• Death of a participant or dependent
• Loss of other group health coverage
