Dependent Premiums and Payments

When are premium payments due?

Your premium payment is due on the first day of each calendar month. To avoid disruption in your Dependents’ coverage, make sure your payment is received by the due date.

Your billing statement and Benefits Manager account will show the applicable premium amount and payment due date.

Important: The Health Plan allows one Late Payment Waiver per Benefit Period for Dependents of Senior Performers. 

How can I pay my premiums?

You can manage your premium payments through Benefits Manager. 

Payment methods:

• Automatic payments: Set up recurring monthly payments (bank or credit/debit card)
• Online payments: Make a payment through Benefits Manager
• Phone payments: Call (800) 777-4013
• Mail: Send a check with your billing coupon to:
